A Tiny Miracle

 Time goes swiftly, it has been six months since I entered the Paediatric/Neonatal intensive care unit (PNICU). Most people may think this is a sad and mystery place. However, I think this place is filled with challenges, love and hope. I would like to share a story of my patient showing the instinct of love between parent and their little fighter.

BB止痛糖水-苦中一點甜24% oral sucrose

一邊傳來絕望的叫聲“屎袋漏咗呀”然後同事叫道“俾定啲甜甜嚟呀”，指的就是24%sucrose(24%蔗糖溶液）。當BB要面對短暫痛楚的程序例如傷口拆線，更換stoma bag,睇ROP(眼睛檢查）等...我地就會俾啲甜甜佢地食，這些甜甜的“糖水”對BB來說具有效的止痛作用！

今日睇到一份journal好有趣😍，美國醫學會雜誌（JAMA)喺今年3月發表一份systemic review and meta-analysis,研究題目是那種藥物治療有最大機會關閉haemodynamically significant PDA（開放性動脈導管）in prem baby(早產兒）。 結果顯示High dose 嘅oral ibruprofen比 Standard dose嘅 IV ibruprofen or indomethacin更有效關閉PDA😮😮😮而採用conservative treatment嘅BB，即唔做相關治療並無增加死亡率或連帶NEC，IVH機會！！！😮😮😮

淺談CO2 monitoring in PICU 

我地監察一個病人在使用呼吸機或額外氧氣時，除了SpO2(氧氣含量）外會很看重他身體的二氧化碳(CO2)含量，因為關係到alveolar ventilation status即肺泡換走二氧化碳的能力！ 身體需要適當的氧氣和二氧化碳濃度去保持血液的酸鹼值（pH)讓細胞正常運作！

CO2 理想值係4-7kPa,同參考病人本身CO2值。

有個bb本身呼吸好差，長期维持CO2在7-9，有時高見15-17😨😨😨😨嚇死人，所以當時aim少過10kPa已經理想，當然最理想是解決背後病因，控制CO2在4-7kPa！

唔好亂咒人"生仔無屎忽"😈😈😈

我地病房耐不耐都會收到無patpat窿嘅仔，佢地正確學名叫Imperforated anus（IA)肛門閉鎖,或者Anorectal malformation 又叫Anal atresia。唔止無pat pat窿先叫IA,連開錯位置，太窄，有fistula(通道連接vagina,urinary, or prostate 去rectum )都叫IA.
